All applicants who wish to apply to stay in Sarawak under Malaysia My Second Home Programme must submit the following documents to One Stop Agency:

1. Letter of Application (Cover letter from applicant)
2. One (1) copy of MM2H Application Form
3. One (1) copy of IM.12 Application Form (Social Visit Pass Form)
   [Note: Applicant and all the dependents have to complete the form individually]
4. Personal Bond Form (affixed with RM10.00 stamp duty by Inland Revenue Board of Malaysia / 2nd Class Magistrate / Commissioner of Oath)
5. BK Form (Sponsor's Particular Form)
6. Certified copy of Passport / Travel Documents (all pages)
   [Note: Copy of previous Passport is required if applicant has renewed his / her Passport in less than one (1) year]
7. Five (5) copies of passport-sized photographs
8. Certified copy of Sponsor's Identity Card
9. Cover Letter from Sponsor
10. Certified copy of medical report and RB II Form
11. Certified copy of Marriage Certificate (if accompanied by spouse)
12. Certified copy of Birth Certificate / Legal Documents (if accompanied by children)
13. Certified copy of Fixed Deposit Statement from any bank / financial institution in Malaysia
    [RM150,000.00 (couple) & RM100,000.00 (Individual)]

OR

Certified copy of latest three (3) months government approved pension funds (for applicants 50 years and above who apply using proof of monthly off-shore income)
